of equipment or switch to a new provider, Cellect Cellular will pay you for those unused wireless devices.

download the PDF form, complete it, and fax it to us at 1-888-807-5640. We will prepare and return a quote based on the information
provided.
Once you receive your quote and accept our offer, we will provide you with a pre-paid shipping label. Payments
will be made by check within 30 days of receiving the equipment.
